tengo una pagina en php de un envio de formulario:

como adapto una a otra. Es decir como le doy la apariencia que tengo en el html al php?

perdonar todo este mogollon. Me acaban de dar con una solucion:

incluir esta linea en el php:

if (mail($para, $asunto, utf8_decode($mensaje), $header)) {
header("Location: pagina_envio_formulario_ok.html");
} else {
echo "Ha habido un error enviando el formulario, <a href=paginadelformulario.html'> volver atras.</a>";
}

y todo solucionado.
