Evitar duplicidad en archivos subidos al servidor

Hola buenos días.
Me interesaría introducir un número aleatorio para evitar duplicidad en archivos subidos al servidor.
¿Dónde tengo que colocarlo? ""substr(md5(uniqid(rand())),0,6)""
Intento con este, pero puede ser otro.
