Hola buenos dias como estan? ya hice todo lo que me habian comentado e incluso monte el formulario con el script y todo, cuando lo voy a probar me da error, voy a pasarles aca el error lo que dice alomejor me pueden ayudar...

Supuestamente esta todo bien pero claro, hay algun detallito por lo que no funciona, porfavor si me pude ayudar.. Muchas gracias :)
